Transforming Care Partnerships – Housing Choices, Housing Cares - Birmingham

Event date

Presentation notes and slides

Description

We are working closely with NHS England to aid the delivery of their programme, ‘Building the right home: transforming care for people with a learning disability, autism or both’ across a number of CCGs and councils; in particular, to help build effective partnerships and provide support in identifying a range of housing pathways.

This free, invite only roundtable event will bring together the key commissioning and partners from each health, housing and care economy and will provide an opportunity to:

  • find out more about the programme
  • participate in facilitated discussions
  • share the learning from all the sites
  • exchange practical ‘what works’ solutions; and
  • better understand the difficulties in implementing the programme and ways to overcome them.
