Realistic evaluation of integrated health and social care for older people in Wales, to promote independence and wellbeing

This two year study by the University of Swansea for the Welsh Government (2011), highlights some potential difficulties and barriers to effective integration; discusses problems with the concepts, definitions and understanding of integration; and identifies examples where integrating services has, or has the potential to significantly improve services and outcomes for older people. While not extra care specific, it makes a strong case for housing related services and improvements to existing stock to enable older people to live well at home.
