Complying with Monitor's integrated care requirements

As the sector regulator for health services in England, Monitor have published an online document to help NHS organisations work across traditional boundaries to better deliver care and support that meets the needs of patients and service users.

There needs to be a common language and a shared understanding of the term integrated care. In particular, they define 'integrated care and support' as follows:

  • The definition of integrated care and support is: "Care and support is integrated when it is person-centred and coordinated".
  • The definition of 'the person-centred narrative is: "I can plan my care with people who work together to understand me and my carer(s), allow me control and bring together services to achieve the outcomes important to me".
