Integration of health and social care: Rising to the challenge

This report from Brunelcare presents the findings from a working group established to compile and publish evidence of how Brunelcare contributes to the integration of Health and Social Care within the South West region.

A key factor in the working group's response was the National Audit Office report published in February 2017, entitled ‘Health and social care integration’. This report explored how the work of Brunelcare supports and evidences the integration of health and social care.

In gathering evidence, the group focused on two specific elements of Brunelcare's performance:

• Reducing hospital admissions
• Reducing the delay to transfer of care from hospital to other settings
