NHS England framework to improve the lives of care home residents

This framework is based on the work of 6 NHS Vanguard sites in England where care homes are working closely with the NHS, local authorities, the voluntary sector, carers and families to optimise the health of their residents.

The documents also makes reference to extra care housing and recognises the importance of joined up commissioning and collaboration to deliver better service integration and meet local health and social care needs. In particular, paragraph 2.19 states:

"Depending on the need identified and personal circumstances, these options might include adaptations to their home, assisted living arrangements, or access to a range of residential or nursing homes."

"Longer term, public and private sector partners could use planning, funding and policy levers to work toward a mix of specialised housing which meets the requirements of those with care and support needs, and general purpose housing which works better for an ageing and diverse population."
