New interactive map shows a wide range of projects preventing delayed discharges from hospital due to a housing issue

This new interactive map developed by the Housing LIN and Foundations, provides an insight into a wide range of projects across England, delivered by providers of housing support and local housing authorities to prevent delayed discharges from hospital due to a housing issue.
