This jointly produced report produced by Nottingham and Nottinghamshire CCG and Nottingham City Homes presents findings on the strengths and areas for improvement of the local work carried out to identify patients in hospital or in the community living in unsuitable housing.

It details the successes of the Housing to Health services and the key factors that had made it successful, as well as how these elements can be applied to other services in other areas. Within the report there are detailed descriptions of how the project worked, case studies showcasing positive impact and a break down of essential requirements for each stage of project implementation.

In the section of the report ‘Potential to Spread’, the factors of replicability of the project elements to other areas and the possible variations in the project that can lead to success in other areas.
