Refreshing the High Impact Change Model

The importance of Housing to improved integration has been added to the influential High Impact Change Model which identifies approaches and service models that can have the greatest impact on reducing delayed discharge.

Originally developed in 2015, it has been refreshed in 2019 with input from a range of partners including the Local Government Association, the Association of Directors of Adult Social Services, NHS England and Improvement, the Department of Health and Social Care, the Ministry of Housing, Communities and Local Government and Think Local Act Personal Partnership, and the Housing LIN.

The model is endorsed by Government and builds on lessons learnt from best practice and promotes a new approach to system resilience, moving away from a focus solely on winter pressures to a year-round approach to support timely hospital discharge resulting in quality outcomes for people. There are nine specific High Impact Changes  with one new one giving specific attention to housing for the first time, with an accompanying maturity index. We are also pleased to see reference to the Housing LIN and signposting to a selection or resources available on our website.
