The NHS Neighbourhood Health Implementation Plan
This welcome pack provides an introduction on neighbourhood health for 16 large-scale, local demonstrator sites. It gives a strategic overview within the context of the NHS 10-year Health Plan, in particular around three key operational shifts: from hospital to community, from sickness to prevention, and from analogue to digital.
It also sets out the local infrastructure required to achieve high impact changes, the partnerships, roles and responsibilities needed to support implementation and types/levels of organisational support available to deliver better health outcomes for local population health. While housing and care at home are not explicitly referenced, the Pack does highlight the need to “co-design care and delivery with communities and practitioners, mobilising and coordinating and strengthening all the assets in the system and community”, as part of a local ‘Compact’.
