Pan-Lancashire Transforming Care Partnership: Supported Living Framework

The Pan-Lancashire Transforming Care Partnership (TCP) is a strategic alliance of four councils in the North West, with eight CCGs and NHS England specialised commissioners.

Led by Lancashire County Council, one of the TCP’s key priorities was to increase the range of support in the community to facilitate safe and effective hospital discharge.

It recognised that a lack of housing solutions was leading to delays in creating support packages in the community and the need for a supported living framework for a strategic approach to developing more flexible housing options.
