'The Role of Housing in Effective Hospital Discharge'. The Chartered Institute of Housing and Skills for Care have drawn together a wide range of case examples of providing examples of effective ways in which housing services have adapted or developed their offer to include support for those leaving hospital. It also examines how offering these services has affected their workforce and their model of support. This publication supports the Memorandum of Understanding to support joint action on improving health through the home. Housing
leaders agreed this memorandum of understanding with the Department of Health, the Department of Communities and Local Government, NHS England and Public Health England, along with other partners to establish the principles and purposes behind more
integrated working.
