New Guide published to support local health and social care systems on discharge to assess models

‘Imagine leaving your home abruptly and never returning to it again…imagine being told that you are moving house tomorrow and you have no control over where you are moving to and how much it will cost’.

A new quick guide has been published which aims to address these scenarios by supporting local health and social care systems to reduce the time people spend in hospital once their acute care has ended and wherever possible, support people to return to their home.

The quick guide offers practical information for both commissioners and providers on discharge to assess and provides top tips and resources to help localities initiate or improve similar systems, including best practice from across the country.

A unique series of open day events are available to find out more about local examples of discharge to assess described within the quick guide.

Five local systems are offering the opportunity to spend time with their clinicians and managers involved in setting up and running discharge to assess - to answer questions and share more information on:

- How their discharge to assess model operates;
- How they got started;
- How they have implemented and maintained changed ways of working; and
- Practical tips learned along the way.

These events are ideal for anybody looking at introducing a discharge to assess model, or enhancing/up scaling an existing model.
