Improving Health and Social Care Through Housing - London

This Forum provides participants with a valuable opportunity to hear a progress update on how best to deliver tailored and integrated housing and healthcare services as part of a preventative approach to improving healthcare outcomes.

Attendees will develop an understanding of how to bridge the gap between contrasting healthcare and housing priorities and hear from innovative best practice examples of joint working on how to reduce pressures on the NHS, avoid service overlap and do so in a financially efficient manner.

  • Providing step down services in temporary accommodation – including for homeless people – for people able to be discharged but without appropriate housing and support at home
  • Supplying information and advice services and telecare/telehealth schemes to help people remain independent.
