Proactive care: Providing care and support for people living at home with moderate or severe frailty

This guidance, published by NHS England, supports a more consistent approach to Proactive Care across England, following the latest evidence and best practice.

The onset or progression of frailty can be slowed by taking a biopsychosocial approach and putting in place personalised and preventative measures that address a person’s range of needs in a timely way, enabling them to live independently and healthier for longer.

This guidance is for integrated care boards (ICBs) and provider organisations involved in the design and delivery of proactive care.

While focusing on supporting people with long-term conditions to live independently at home, this guidance primarily looks at health and care interventions that enable the continuity of appropriate care. Surprisingly, there is no information about how housing improvements can help facilitate care at home or a hospital discharge, for example, access to aids, adaptations, equipment and/or technology.
